MF-01	Per-doctor slot duration not configurable in UI — `practitioner_schedules.slot_duration` and `practitioner_roles.default_slot_duration` exist in DB (default 15 min). Both `AvailabilityService` and the booking modal read these values. But the availability settings page at `/cms/settings/practiceSettings/availability` has no UI to change them. All doctors use 15-min slots with no override.	Missing UI
